What is an Ayushman Card
The Ayushman card is a special type of identity card that is issued under the Ayushman Bharat Yojana. This card gives you the benefit of free treatment in government hospitals.
How to get an Ayushman card
To get an Ayushman card, you have to follow the following steps:
Eligibility check: First of all, you have to check whether you are eligible for Ayushman Bharat Yojana or not. The eligibility criteria for the scheme may vary from state to state, so you will need to check your state’s rules.
Visit the nearest Jan Seva Kendra: Once you confirm eligibility, you will need to visit your nearest Jan Seva Kendra.
Fill out the application form: At the center, you will be given the Ayushman card application form. You will have to fill in your details and the required documents in this form.
Submit documents: You will need to submit the following documents along with the application form:
Aadhaar card
Address proof (ration card, passport, driving license, etc.)
Family income certificate
Passport size photo
Verification of documents: The documents you submit will be verified.
Issuance of Ayushman card: If your documents are found to be valid, you will be issued an Ayushman card.
How to use Ayushman card
To use the Ayushman card, you will need to visit the Mitra Help Desk at a government hospital.
You will have to show your Ayushman card and apply for treatment. The hospital staff will arrange for your treatment and you will not have to pay any fee.
Benefits of Ayushman Bharat Yojana
Under the Ayushman Bharat Yojana, you get the benefit of free treatment in government hospitals. Health insurance cover of up to Rs 5 lakh is provided under the scheme. This scheme is applicable to all the members of the family.
